The whole show hangs on a single fact laid out in the pilot: Dan Scott fathered two boys in the same year, with two different women, and raised only one. Lucas and Nathan share a face, a jersey and a last name before they share a word. In 1988–89, Dan Scott got two women pregnant: his high-school girlfriend Karen, then his college girlfriend Deb, whom he married. He raised Deb's boy, Nathan, in a mansion — and pretended Karen's boy, Lucas, didn't exist, though the whole town knew. The pilot begins the day the half-brothers are forced onto the same basketball team.
Dan's older brother Keith — the mechanic — helped Karen raise Lucas and spent seventeen years quietly in love with her. He's the father figure of the show, which is exactly why his murder in the Season 3 school-shooting episode, and the reveal that Dan pulled the trigger, is the series' point of no return.
Peyton grows up thinking widowed sea captain Larry Sawyer is her only parent. Then Ellie Harp arrives in Season 2 — her birth mother, who gave her up and dies of cancer in Season 3 — and Season 6 adds Mick Wolf, the washed-up rocker who turns out to be her birth father. Adoptive dad solid line, birth parents dashed.
Best friends Brooke and Peyton both love Lucas, and the show runs on it for years — dated her, then her, then her again. Peyton is the endgame: they marry in Season 6 and drive out of Tree Hill with baby Sawyer. Brooke gets the truer happy ending at home, with Julian and the twins.
Nathan starts the series as Lucas's bully; his tutor Haley — Lucas's best friend — humanizes him, and they marry as 16-year-old juniors in Season 1. Every other couple wobbles; these two just keep going: Jamie arrives on graduation day, Lydia in Season 8, and the marriage outlasts the whole run.
After killing Keith, Dan loses everything slowly: prison, a heart that needs replacing, a family that won't have him. Season 9 gives him his ending — he storms the kidnappers holding Nathan, takes a bullet doing it, and dies with Keith's ghost telling him he's forgiven. The show's best-argued redemption arc.
One surname on three-quarters of the chart, a girl named Sawyer, a baby named Sawyer, and a Davis on each side of the family. Keep these apart:
The half-brothers. Lucas = Karen's son, raised poor over the café, broody, writes. Nathan = Deb's son, raised rich by Dan, cocky, dunks. Same father, same year, same team — strangers until the pilot.
The two mothers. Karen Roe = café owner, never married Dan, raised Lucas solo. Deb Lee = married Dan, raised Nathan in the big house, and spends most seasons getting free of him.
The Scott brothers. Keith = older, kind, a mechanic, the one who actually showed up for Lucas. Dan = younger, ruthless, car-dealership king and later mayor. One murders the other in Season 3.
Peyton's three parents. Larry = the adoptive dad who raised her. Ellie = birth mother, appears S2, dies S3. Mick = birth father, revealed S6. Her adoptive mom, Anna, died in a car crash before the series.
Peyton Sawyer = the artist with the curls. Sawyer Scott = her daughter with Lucas, born in the Season 6 finale and given her mom's maiden name — plus middle name Brooke, for the best friend.
Brooke Davis = the fashion mogul. Davis Baker = her twin son, first name from her maiden name. His brother Jude got the normal name. Their dad Julian was born a Norris — he took his mother's name, Baker.
Nathan and Haley's two. Jamie (James Lucas Scott) = the scene-stealing kid of the later seasons, named for his uncle. Lydia = his little sister, born in Season 8, named for Haley's mother.
The surnames tell the custody story. Roe = Karen (and Lily's middle name). Lee = Deb, who dropped “Scott” after the divorce. If someone's called Scott, they're Dan or Keith's line — or married into it like Haley.

Coach Brian “Whitey” Durham has run the Ravens since he coached Dan and Keith themselves — the gruff conscience of the show, refereeing the Scott feud from the bench.

Marvin “Mouth” — River Court play-by-play kid turned real sportscaster, and the friend group's most loyal member. Marries Millie in the finale's flash-forward.

Antoine “Skills” — Lucas's playground crew from the River Court, folded into everyone's family by the end; he even helps coach the Ravens in the later seasons.

Nathan's cocky young teammate and little Jamie's hero in Season 5 — shot dead in a convenience-store robbery early in Season 6, the loss that shadows that whole season.

The musician who talks about Chris Keller in the third person — cheerful chaos for the Season 2–3 couples, back unrepentant for Season 9.
