Meredith has two half-sisters — one from each parent. It gets worse from there.

Grey Sloan employs hundreds of surgeons, but the show's real maze is the family knotted around Meredith Grey: her mother's affair with the Chief, her father's second family, a secret sister given up for adoption, and a husband whose own sister works down the hall. This chart draws only that knot. Teal borders are the Greys, blue the Shepherds, amber the Webber–Avery–Fox axis — and the dashed line is the affair that quietly rewired all three.

Grey Sloan's org chart is simple. Its family chart is not — and every big reveal in the series lives on the lines above.

The core confusion

Three Grey sisters — no two with the same parents

One sentence, three sisters: Meredith = Ellis + Thatcher, Lexie = Thatcher + Susan, Maggie = Ellis + Richard. Lexie and Maggie are each Meredith's half-sister — through opposite parents — so the two of them are not related to each other at all (and never met: Lexie died two seasons before Maggie arrived). Molly is Lexie's full sister, off raising a family.

The affair

Ellis and Richard, the show's original sin

As young residents, married-to-other-people Ellis Grey and Richard Webber had an affair. He stayed with Adele; Ellis moved away — secretly pregnant. That baby, Maggie Pierce, was adopted out, grew up a prodigy, and turned up at Grey Sloan as head of cardio in Season 10, where the truth landed on Richard and Meredith in turn. The dashed line is thirty years of secret.

The Shepherds

Two Dr. Shepherds in one OR wing

Derek is one of five Shepherd siblings — four of them are doctors. The one who matters here is his youngest sister Amelia, also a neurosurgeon, who joins the same hospital and inherits his department after his death. When someone pages "Dr. Shepherd," check the season before deciding who turns around.

Sofia's three parents

One kid: Torres + Sloan + Robbins

Sofia was conceived by Callie Torres and Mark Sloan — while Callie was with Arizona Robbins, who raised Sofia as her own. Three parents, hence the show's most heroic baby name: Sofia Robbin Sloan Torres. Mark died in Season 9; her mothers married, divorced, and both eventually moved to New York.

Why Meredith's kids sound familiar

All three of Meredith and Derek's children are named for people elsewhere on this page: Zola (adopted first), Bailey — for Miranda Bailey, the resident who trained Meredith — and Ellis, for the mother Meredith spent a lifetime untangling. The show does it too: the hospital itself is renamed for two of its dead.

The marriage loop

Owen Hunt, serial in-law

Owen connects three corners of the chart by marriage alone: first Cristina Yang (Meredith's person), then Amelia Shepherd (Derek's sister), then Teddy Altman (his old army friend, mother of his daughter Allison). That's why his gold lines wander further than anyone's.

The name decoder

The show reuses its favorite names with total confidence. Keep these apart:

Ellis vs. Ellis

Ellis Grey = Meredith's mother, the surgical legend who died in Season 3 and haunts the whole series. Ellis Shepherd = Meredith's youngest child, named after her. When Meredith talks to "Ellis," check whether it's a flashback.

Bailey vs. Bailey

Miranda Bailey = the surgeon ("the Nazi" of Season 1, the Chief by Season 12). Bailey Shepherd = Meredith's son, named in her honor. One runs the hospital; one is a kid in it.

Lexie vs. Maggie vs. Molly

All younger sisters of Meredith, none interchangeable. Lexie — Thatcher & Susan's daughter, surgeon, dies in the plane crash. Maggie — Ellis & Richard's secret daughter, cardio chief. Molly — Lexie's full sister, a civilian who moved away in Season 2.

Grey vs. Grey Sloan

The hospital's names: Seattle Grace → Seattle Grace Mercy West → Grey Sloan Memorial. The rename honors plane-crash victims Lexie Grey and Mark Sloan — it is not named after Meredith, however it sounds.

Shepherd vs. Shepherd

Derek (seasons 1–11) and his sister Amelia (season 10 onward) — both "Dr. Shepherd," both neurosurgeons, same hospital, back to back for a season and a half.

Avery vs. Fox

Same dynasty, rebranded. The Harper Avery award and foundation — named for Jackson's grandfather — became the Catherine Fox Foundation in Season 14, after Harper's scandal broke. Jackson stayed an Avery; Catherine became a Fox.

"The Chief"

Bailey, Owen, Teddy, and Meredith all hold the Chief of Surgery job at some point — but unqualified, "the Chief" still means Richard Webber, who held it for the first eight seasons and never really stopped.

The post-it

Meredith and Derek's real wedding, Season 5: vows written on a blue post-it note, stuck to a wall. Legally they married later (Season 7, for Zola's adoption) — but the post-it is the marriage everyone means.

Not blood — but closer than family

The found family: Meredith's people, and the three-parent household the chart above can't hold.
